Psychotoxicis a first-person shooter where the player controls an Angela Prophet, whose mission is simply to save the world from the Fourth Horseman who can bring Armageddon to end all existence. Angela, with a little help from some friends, must travel through futuristic cities and fantastical Dream Worlds, shooting many enemies in the process to stop evil from destroying everything.

The game covers two different types of levels: the real world, which is a futuristic city with normal weapons and enemies, and then there is a dream world where not only the weapons but also the enemies and the visuals are sometimes quite strange. There are 19 weapons in total, meaning that both the dream world and the real world have around 10 firearms each. They range from normal pistols and rifles to more fantastical ones that the dream world has to offer.

The basic gameplay is 3D shooting through Angela's eyes as she progresses through levels, shooting people (and monsters), picking up things, opening doors, and talking to people. She occasionally arrives in the Dream World, where each level is more spooky than the last, from cartoon characters to gothic towns and environments very similar to hell.
