I Am Aliveallows you to play as a man who returns to his hometown after a massive earthquake. The entire metropolis was completely destroyed. The protagonist arrives at his own house in search of his wife and his young daughter who left the place almost a year ago, along with a wave of refugees heading to a shelter.

In I Am Alive you play as a man who, armed only with a gun and a bow with little ammunition, has to fight to survive in a world governed by the principle of supremacy of the strong. More important than physical strength is cunning and foresight. The game takes the form of a linear platform with elements of an adventure game. He also looks for items that can help him survive. The hero's status is represented by two factors: stamina and health. Therefore, when exploring, he should carry something to renew his strength, such as energy drinks and inhalants. Health can be restored with pain relievers and medical kits.

The game features 3D graphics in different shades of gray. Only occasionally do you come across colorful elements. The design is intended to reinforce the metaphorical presentation of the city as a ruined, dirty and hopeless place. Most of the time you watch your character from behind. Only when you aim a gun or a bow does the perspective change to first person.
